Working with colleagues who love what they do and are deeply committed to our Mission, you too can be part of our transformation across the enterprise.   Quality Improvement RN Full-Time   Overview:    The Quality Improvement RN will develop a performance improvement quality program to support the nursing clinical staff achieve the highest level of quality and patient safety by ensuring HSS meets or exceeds compliance with TJC, NYS DOH, CMS regulations and Magnet benchmarks. The role will collaborate with the nursing leadership team and Quality and Accreditation team to coordinate activities and develop quality improvement projects; and provide support for nursing staff and multidisciplinary teams undertaking improvement initiatives. The quality improvement RN will provide and analyze data, anticipate programmatic changes, inform organizational and clinical leadership of such changes, forecast organizational performance, and coordinate performance improvement initiatives to meet the goals. The role supports evidence-based practice by assisting nursing staff in the development of process standards that are consistent with evidence-based literature, regulatory requirements, and pertinent professional nursing standards. This role will act as a departmental liaison between the Department of Nursing and other hospital departments to drive collaboration between interprofessional partners to improve quality care across the enterprise.   Responsibilities include:   - Annual evaluation and report of the Nursing quality and performance improvement plans - Occurrence reviews, including Patient Event Debriefs, RCAs, FMEAs, and corrective action plans with Nursing leadership team and the Quality and Accreditation team. - Analyzes data, evaluates outcomes, and supports the development of action plans to improve practice. - Assists in improvement to ensure the right methodologies are utilized, including Lean and Six Sigma, A3 Problem Solving, AND Plan-Do-Study-Act (PDSA). - Monitors regulatory compliance. Coordinate activities to ensure the effective implementation and sustainability. - Identifying opportunities for improvement and the development and implementation of improvement strategies - Assist with benchmarking activities through the National Database for Nursing Quality Indicators (NDNQI). - Provides project management support for quality improvement teams including arranging meetings, providing minutes, managing project timelines, and preparing reports. Participates in development of presentations, reports, and publications. - Assessment of the learning needs of nursing staff as they pertain to quality and performance improvement; assists with the provision of education and mentoring in the methods and tools of rapid cycle and continuous quality improvement. - Develops tools to audit and monitor outcomes at the unit level. - Manages all phases of a Nurse Sensitive Indicator (NSI) dashboard, including data collection, evaluation, and presentation of data to the clinical staff in partnership with unit-based nursing leadership - Coordinates and serves as the contact person for the NDNQI (National Database of Nursing Quality Indicators) data collection, reporting, analysis, and dissemination of results. - Coordinate nursing peer review activities.  Passion - Commitment to achieving higher levels of performance for patients, staff, and the organization - Demonstrates focus and alignment with the organizational goals - Displays the desire to be a lifelong learner - Driven to achieve result Teamwork - Mentors and coaches all members of the team in quality improvement methodologies and performance improvement project management - Demonstrates necessary skills for team building, open communication and promotion of a collegial atmosphere - Collaborates with the interprofessional team Excellence - Collaborates with the team to drive quality initiatives - Partners with Patient Care Directors regarding data related to clinical practice, staff communication and change facilitation - Active engagement, council membership and serves as co-chair on councils - Utilizes performance improvement model (Plan-Do-Study-Act) to improve patient outcomes  - Critical Thinking: Analyzes problems, issues and opportunities. Working with colleagues who love what they do and are deeply committed to our Mission, you too can be part of our transformation across the enterprise.   Regional Clinical Education Specialist (CES)-Ambulatory  Four (10) shifts weekdays -On site    Clinical Education Specialist – Ambulatory   The CES will be responsible for implementing theory based professional practices evaluating and promoting best care plans for patients and their families. The role will serve as a resource and mentor to all clinical roles at the ambulatory sites ensuring curriculum, policies and procedures remain up to date and in compliance with regulatory agencies. The CES will generate data-driven plans that take both quality and cost of care into account using appropriate research practices and methodologies to get accurate data and drive practical change to improve clinical practice. The CES provides both clinical leadership and education through ongoing clinical evaluation with the nursing staff. The CES will support orientation and competency training for all clinical roles in the ambulatory setting. The CES supplies analytical support to the nursing leadership to plan and predict education needs based upon patient and nurse performance data and direct observation of nursing practice. The CES possesses mastery of clinical decision making, critical thinking, knowledge acquisition and dissemination, problem identification and identification of safety issues in practice. The CES Understands the Magnet Program and integration of Magnet standards into the nursing department infrastructure. The CES supports the shared governance model. Utilizes American Nurses Credentialing Center Commission on Accreditation/ Continuing Education Approval Program (ANCC-COA) criteria to assess needs, plan, implement and evaluate continuing nursing education activities which qualify for continuing education - Critical Thinking: Analyzes problems, issues and opportunities.
