Autonomously and in collaboration with healthcare professionals and other individuals, the NP provides a full range of primary, acute, and specialty healthcare services, including: - Diagnosing and treating acute and chronic conditions such as diabetes, high blood pressure, infections, and injuries - Ordering, performing and interpreting diagnostic tests such as lab work and x-rays - Prescribing medications and other treatments - Managing patients' overall care - Counseling - Educating patients on disease prevention and positive health and lifestyle choices HSS NPs serve pediatric and adult patients in an extensive range of practice settings throughout the hospital and its local and regional ambulatory sites. Their specialty and subspecialty areas include: Adult Reconstruction and Joint Replacement (ARJR), Anesthesia (PACU/OSCU/SDU), Anticoagulation, Cardiology, Diabetes Care, Intensive Care, Metabolic Bone, Occupational Health, Pain Management, Pediatric Orthopedics, Post-Operative Care, Radiology, Spine, Trauma, Wound Care, and private practice (physician and surgeon offices).   The HSS cardiology nurse practitioner supports the management of patients with pacemakers and Implantable Cardioverter-Defibrillators (ICDs) in the perioperative setting. The NP evaluates patients pre-operatively in the holding area and post-operatively in the post-anesthesia care unit (PACU); also available intraoperatively, when necessary, to troubleshoot device issues. NPs bill for their services, when applicable. The Cardiology NP role: - Performs chart review and develops a plan on device alteration depending on type of surgery, anatomical location of surgery, cardiac history, reason for device implant, etc. - Orders appropriate diagnostic tests and evaluating results; manages medications - Manages patients with implanted cardiac devices (e.g., pacemakers and ICDs) - Interrogates, alters, and programs cardiac devices - Evaluates and treats patients with concurrent cardiac conditions; collaborates with hospitalists on cardiac-related issues (e.g., congestive heart failure, fluid overload, atrial fibrillation, other heart rhythm disorders) - Develops and implements short and long range plans of care - Serves as a resource for clinical staff regarding cardiac devices This NP position is based at the main hospital in the Upper East Side of Manhattan (New York, NY). This is a full time position, working 12 hour days (three per week).   This position requires a motivated NP who is able to function autonomously within established practice protocols and a collaborative agreement.

The Occupational Health NP role: - Performs pre-employment physical exams; collaborates with Human Resources (HR) to clear individuals to begin employment - health history, planting a tuberculin skin test (TST), ordering labs, reviewing immunization schedules, obtaining urine for drug screen, etc. - Orders and interprets x-rays (assessing positive (+) Tuberculin test) - Orders laboratory and other diagnostic testing; interprets results - titers: Measles, Mumps, Rubella Varicella (MMRV), Hepatitis A/B/C, liver function tests (LFTs), CBC with diff; Quantiferon tests - Administers vaccinations and reviews vaccination records (e.g., Influenza, Tdap, Hep A/B, Varicella, MMR, COVID) - First line of contact for employee related health issues related to COVID-19 including exposure and return to work guidance - Develops treatment plans and documents patient encounters in the Electronic Health Record (EHR= Agility) - Evaluates and follows up on work-related injuries; collaborates with Risk Management, Case Management, and department managers for accommodations to return to work (RTW) - Responds, treats, and follows up with occupational exposures (e.g., needle sticks, blood borne pathogen exposures (BBPEs)) - Evaluates episodic sick visits, makes referrals as needed and instructs employees on self-care/management to improve level of health - Assess employees to ensure employee is “fit for duty” after being treated for work related injuries, illness, communicable disease, etc. - Responds to emergencies and assists employees with serious medical conditions - Assists with counseling, referral, and navigating complex self-care issues and life events for physical conditions such as: - hypertension, diabetes, obesity, pregnancy - psycho-social and mental health issues (e.g., anxiety, depression, bi-polar disorder) - social issues (e.g., alcohol abuse, substance abuse, domestic violence) This NP position is based at the main hospital in the Upper East Side of Manhattan (New York, NY). Establishes a presumptive diagnosis, performs the general work up of the patient and maintains responsibility, in collaboration with the physician, for the management of problems facing the patient and families after diagnosis. - Makes complete entries in the inpatient and outpatient electronic medical record, places orders, and records progress notes in EPIC. - Initiates appropriate laboratory radiological, and special examinations or tests required for the evaluation of complaints and symptoms - Communicates with patients/parents/guardians/referring physicians by phone and or written letter regarding their problem. - Provides counseling and instruction to patients and their families regarding the patient’s health related problems. - Develops a comprehensive plan of care that includes interventions to promote, maintain and improve health and prevent illness and injury. - Documents summaries of the clinic visit and communicates with referring physicians via electronic medical records. - Maintains CPR certification - Prescribes medications or relays telephone orders for institution or renewal of medications and treatment that may be accepted by a registered nurse and or registered pharmacist. - Manages medical emergencies and initiates appropriate therapy in collaboration with supervising physician - Provides follow up in health maintenance care including medication adjustments - Assists in training health personnel - Performs on call duties such as performing inpatient rounds and consults, answering patient and physician phone calls after hours and/or on week ends and evaluating patients on an emergency basis in the inpatient setting - Participates in the appropriate internal continuing medical education programs - Meets CME requirements - Assists clinic team in multidisciplinary settings facilitating care and implementing programs as a result of those visits. - Provides consultation to medical and nursing staff while offering specialized skills and knowledge to assist with problem solving. - Promotes patient satisfaction and integrated principles of caring into patient services - Performs sled directed, highly autonomous work and purposefully seeks necessary knowledge and skills to develop and maintain competency in the role. - Serves as a clinical resource for nurses and new staff and may provide service specific orientation pertinent to clinical education. - The NYP may collaborate on research, quality assurance programs, and assist with the formation of protocols that will be applied to clinical practice or further the educational and clinical expertise. - Performs all other duties as required or assigned by collaborating physicians or the Chair of the Department.

Follows up on plans formulated on rounds. - Discharges patients with Attending approval, when appropriate. - Assists the supervising physician in coordinating management plan. - Monitors and evaluates the patient’s treatment and health care plan for effectiveness and modifies as necessary. - Documents all assessments, findings, progress and care provided in the patient’s medical record. - Refers relevant clinical issues to the supervising physician. - Works collaboratively with all nursing and other hospital staff as required. - Adheres to all practice standards outlined by Hospital Administration and governing regulatory bodies.   Department Requirements: - Displays a positive attitude toward work assignments, performance feedback, and interactions with co-workers, patients, families, and visitors. - Willingly adheres to Department and Hospital policies and procedures. - Maintains computer literacy, including proficiency in Outlook email and E-learning. - Attends Department meetings regularly and punctually.   Qualifications: - Bachelors Degree - Graduate of CAAHEP accredited Physician Assistant Training Program - Current New York State registration  - ACLS and BLS certification - Current NCCPA and PANRE Certificate and continuing education and timely recertification as required - Current Infection Control Certificate - Prior critical care experience preferred   Working Conditions: Physical Requirements Physical: Continuously stand/walk or lift/handle/carry material or equipment of moderate weight. Environmental: Located in an indoor area with frequent exposure to mild physical discomfort from dust, fumes, temperature, and noise. Hazards: OSHA Category 1: Tasks that involve exposure to blood, body fluids, tissues, and other potentially infectious materials.
