The goal of the Geriatric Trauma Program is to provide optimal care to our geriatric patients through clinical activities, education, and quality assurance. The coordinator will collaborate with various departments across the hospital including but not limited to, clinical leadership, faculty, medical staff, various administrative departments, appropriate regulatory agencies, residents, and fellows.   Specific responsibilities will include: - Coordinate and direct patient care to ensure patients’ needs are met and hospital policy is followed - Post-operative patient surveillance including: - Call patients and/or patient caregiver on post-op day 1 and 3 - Medication reconciliation - Wound management - Coordinate appropriate follow-up appointments prior to discharge - Oversee weight bearing and range-of-motion restrictions - Facilitate and/or participate in outpatient evaluations in lieu of ED evaluation for medical/surgical issues - Ensure compliance with in-hospital policies, procedures, and protocols including: - Weill Cornell DVT prophylaxis protocols - Weill Cornell anesthesia multimodal pain protocols - Ensure data collection for hospital and service dashboards and registries - Infection control and risk management - System wide pathways including: - Social work consults - PT evaluations - Quality Assurance monitoring - Monitor and assess Geriatric Fracture Dashboard - Track quality metrics for all associated services (Ortho, MOTS, Social Work, PT, Anesthesia) - Monitor patient length of stay - Identify system issues - Consult with other hospital departments as appropriate to collaborate in patient care - Work with the Chief of Orthopaedic Trauma to implement and revise (as needed) clinical Requirements: Bachelor’s degree from an accredited PA program. Masters preferred. Current NYS Physician Assistant license and registration. NCCPA board certification   or   Master’s degree from an accredited school of nursing. Current NYS License as a Nurse Practitioner and RN. Board certification as a Nurse Practitioner. RNFA preferred. The Nurse Practitioner is required to enroll in RNFA program within 6 months of employment. Dr. Danyal H. Nawabi is an Orthopedic Surgeon on the Sports Medicine and Shoulder Service at Hospital for Special Surgery. He is a specialist in the fields of knee, hip, and shoulder surgery, performing both arthroscopic surgery and joint replacement. Serving as an assistant to Dr. Nawabi, duties will include assisting with patient evaluations, treatments, patient and family education, and first assist in the operating room.  You will interact closely with patients and families and be an integral part of developing great patient and collegial relationships.    Competencies/Duties: - Perform complete and accurate history and physical exams. - Serve as 1st/2nd assistant to surgeon in operating room. - Conduct outpatient evaluation, treatment and follow-up of surgical patients as directed by the supervising physician. - Provide post-operative and discharge education to patients and family members with regards to safety issues when applicable (e.g., medication, post-operative precautions, use of equipment, etc.). - Communicate effectively and appropriately when educating patient/family with regard to their medical condition and/or hospital course. - Document progress patient has made toward desired outcomes. - Utilize the computerized order communication system, including transcription of medical orders, retrieval of test results and documentation of results. - Maintain clear and legible history and physical documentation and all other written reports. - Work independently on assigned tasks with minimal supervision. - Promote cooperation through teamwork. - Must be able to travel to NYC and Long Island offices Qualifications:   Candidate must have New York State PA Licensing.   Graduation from an accredited PA program, current NY licensure, BLS/ACLS certification and NCCPA board certification are required.  Previous experience as a neurosurgical PA highly desired.   Also, the candidate must have current Infection Control Certification and have completed a child abuse recognition course. The Physician Assistant Department is a leader in provider clinical operations working closely with the staff to ensure outstanding patient care and a supportive environment for our academic mission. - Management of medical/ surgical issues  for our inpatient population - Admission History & Physicals - Post-operative checks with neurologic assessments - Triage and prioritize patient issues including medical and surgical issues, discharges, etc. - Complete Emergency Admissions when applicable - Collaborate with surgeon, internist and/or hospitalists, specialists and ancillary staff regarding patient care - Participate in the huddles and interdisciplinary rounds as applicable - Outpatient clinic coverage for select services
