The Case Manager applies clinical expertise and core knowledge domains of case management into daily practice. Data is interpolated from patient/ family assessments, the interdisciplinary care team and medical record to evaluate the patient's inpatient plan of care, progress along clinical pathways and care guidelines and discharge plan. Collaborates with the patient/ family, interdisciplinary care team, medical staff, payers, and post discharge providers to facilitate and coordinate patient specific evidenced-based care. Patient care is transitioned through communication and appropriate referral to the next provider of care.   Requirements   The qualified candidate will possess a minimum of 2 to 3 years clinical nursing experience coupled with knowledge of case management, with emphasis on discharge planning. The ability to problem solve, multi-task and manage projects independently is a must. Knowledge of regulatory and insurance standards and protocols as well as proficiency in utilizing evidence based guidelines.  Current NYS RN licensure and Bachelor's degree in Nursing or related field is required. CCM certification highly desirable.

This position will report to the Director, Clinical Data Core within Research Administration.   Essential tasks/responsibilities: - Review study proposals to determine data requirements and assist with development of data collection and/or data extraction plans to optimize data quality and statistical efficiency. - Acquire data from primary or secondary data sources - Evaluate data to discover trends or patterns that may impact data quality and interpretation and work with others to identify potential solutions - Perform data cleaning and management for clinical research databases and datasets. - Build and improve on statistical programming code to make data processing, analysis and presentation more efficient and reproducible. - Create analytic datasets from clinical data and large administrative datasets (e.g. hospital discharge databases) - Generation of tables and graphics for reports, presentations and publications - Contribute written sections for IRB applications, grant applications, and manuscripts for assigned projects. - Perform intermediate to expert level statistical queries, analyses, and evaluation. - Communicate regularly with researchers, clinicians, and other hospital personnel to effectively manage multiple, concurrent research projects. - Maintain current knowledge of data sources within the institution and data available from secondary sources (e.g. Census data) - Stay informed of statistical techniques as applied to musculoskeletal research issues by reading journals and/or attending courses/programming meetings. - Additional duties as assigned.

You will be relied upon to assist Arabic-speaking patients and family members with the completion of required forms and surveys. You will also conduct structured phone interviews with Arabic-Speaking patients and maintain statistics on interpreter services provided. The Arabic Interpreter must be flexible, able to work well under pressure, multi- task assignments, and react quickly to solve complex linguistic and ethical problems as they arise. The interpreter should interpret in an accurate and complete manner; and should follow the Code of Ethics and the National Standards of Practice for Medical Interpreters. The interpreter should have an extended knowledge of specialty medical terminology and be able to perform independently in a fast pace environment. Medical interpreters will translate written materials from source to target language in a precise way. Interpreters will effectively identify cultural differences as potential communication barriers that could interfere with the exchange of information between providers, patients and their families.   Qualification:   The successful candidate must have a minimum of 40 hours of coursework/certification for interpreting in the healthcare profession (i.e. – Culture Smart or Bridging the Gap). Minimum 2-5 years of translation/interpretation field work experience required, experience working in healthcare settings. Written and oral proficiency in English and Arabic language also required. Well versed on code of ethics and protocols for interpreting in a healthcare facility is essential.   Must be proficient in Microsoft Office.   Bachelor’s degree required.

Professional Development Director (PD Director) Full-Time Overview: Responsible for the organizational, educational and clinical development of all In-Patient, Out-Patient, Regional, Peri-Operative  and Post Anesthesia Care Units consistent with the philosophy and goals of Hospital for Special Surgery, the department of Nursing, the ANA Code of Ethics and regulatory bodies. The candidate must have knowledge of Magnet environment and regulatory standards; strong management ability and problem-solving skills; ability to manage multiple projects and priorities simultaneously. The candidate will utilize transformational leadership in partnership with employees to develop goals and objectives consistent with organizational mission and philosophy.   Roles & Responsibilities: -  The PD Director will plan, coordinate, direct and evaluate education and professional development of nursing staff through evidence based best practice. The PD Director will work in partnership with all leaders in planning and implementing education strategies aligned with the core mission and vision of HSS. He/She will direct and coordinate all educational offerings provided by the Division and to the professional community. The candidate will create, foster and encourage a climate of life-long learning and a structure that promotes continuous professional development and mentoring of the staff. - The PD Director will oversee the orientation program for all new employees and incumbents in the Division. They will oversee the competency program for the clinical staff to enhance professional and ancillary competence. Will be responsible for the Nursing Residency and Nursing Fellowship program. The PD director will serve as a Primary Planner for the CNE program at HSS. - The PD Director will participate / serve as a chair in key organizational and departmental committees and councils. The candidate will participate in the strategic planning structures and processes with the senior executive leadership team and clinical leaders. - The PD director will coordinate and conduct periodic needs assessments to maintain timeliness and pertinence of programs. They will evaluate the delivery and effect of nursing education on a systematic and consistent basis providing analysis and proposals for sustaining or improving quality of programs. The PD Director will serve as a consultant to all clinical departments to develop and maintain new clinical practice and competencies as we continue to grow the enterprise and services we offer. - The PD Director will coordinate and foster academic affiliations with schools of nursing for clinical/administrative student placements and student/faculty research sites. Evaluates program/student placement effectiveness on an ongoing basis. Fosters good relationship between hospital and their educational institutions. - The PD Director will Direct and collaborates in the development and evaluation of care delivery education and planned change. Provides leadership and coordination of all education activities required to maintain ANCC Magnet status. Partners with nursing leadership to directs efforts toward interdisciplinary practice improvement and patient safety based upon nursing peer review and quality monitoring findings. - Effectively allocates and schedules resources. Able to project and appropriately staff the unit/department to effectively function. Utilizes resources, both labor and non-labor to maximize efficiency and quality. Adheres to department budgetary guidelines and targets. Oversees the clinical education specialist team and the members of the Professional Development Department to meet the needs of the organization. The PD director will develop, manage and evaluate direct reports. - The PD Director will follow employee retention techniques: effective selection and interviewing, recognition, fair and timely discipline, and ongoing performance feedback. Provides coaching and mentoring. Utilizes best practices for reward and recognition. Interacts constructively, tactfully, and diplomatically with others and resolves conflicts effectively. Demonstrates cultural competency and ability to maintain a diverse workforce. - The PD Director will be responsible for the maintenance of data in the learning management system, education records, competency training, certification rates and licensure for the clinical staff in the Division of Nursing. - Participates in Nursing Quality Improvement activities. Utilizes continuous quality improvement in all work situations with measurable outcomes. Seeks feedback and recommendations from others to improve the quality, processes, and services.
