The Patient Access Specialist is an expert in customer service and ensuring patient needs are met at the end of the visit. SW Coordinator, Latino Outreach RA Support and Education Hybrid Part-Time      An exciting opportunity to develop, implement and facilitate an innovative bi-lingual psychoeducational initiative for people with Rheumatoid Arthritis (RA) in the  Latino/a/x/ Hispanic community. Coordinator will conduct patient-centered research, needs assessments, program evaluations and outreach to inform the launch of this program.  RA is a chronic systemic illness which can lead to significant disability and this population is at higher risk for poor outcomes.  This innovative program seeks to addresses culturally relevant psychosocial-educational concerns of the Latino/a/x/Hispanic RA community and their loved ones, to support strategies for coping and managing this condition.   Major Responsibilities: - Conducts literature reviews, interviews and research with key informants internally and within the community re: culturally relevant targeted outreach to Latino/Hispanic communities to develop culturally relevant content of curriculum and lectures. - Develops, coordinates and facilitates an on-going series of culturally tailored support and education programs - Conducts internal and external outreach related to program promotion - Establishes and cultivates strong collaborative relationships with Community Partners - Creates and conducts needs assessments and program evaluations - Creates formal analysis of outcomes to inform program of future development   Reports to: - Senior Manager, Inflammatory Arthritis Support and Education Programs, Department of Social Work Programs.   Qualifications: - The successful candidate must be bi-lingual (Spanish/English) and have earned an LMSW or LCSW with at least 2 years post-graduate experience. Highly qualified exceptional candidates with less experience will be considered. Self-directed,  highly committed professional with excellent clinical, program development,  and community outreach experience, with excellent written and communication skills required.  Experience in group facilitation required. Experience in a healthcare setting and/or working with people with chronic illness highly desirable. Experience and/or knowledge of qualitative research preferred, including data collection and management. Microsoft Office skills required.

The goal of the Geriatric Trauma Program is to provide optimal care to our geriatric patients through clinical activities, education, and quality assurance. The coordinator will collaborate with various departments across the hospital including but not limited to, clinical leadership, faculty, medical staff, various administrative departments, appropriate regulatory agencies, residents, and fellows.   Specific responsibilities will include: - Coordinate and direct patient care to ensure patients’ needs are met and hospital policy is followed - Post-operative patient surveillance including: - Call patients and/or patient caregiver on post-op day 1 and 3 - Medication reconciliation - Wound management - Coordinate appropriate follow-up appointments prior to discharge - Oversee weight bearing and range-of-motion restrictions - Facilitate and/or participate in outpatient evaluations in lieu of ED evaluation for medical/surgical issues - Ensure compliance with in-hospital policies, procedures, and protocols including: - Weill Cornell DVT prophylaxis protocols - Weill Cornell anesthesia multimodal pain protocols - Ensure data collection for hospital and service dashboards and registries - Infection control and risk management - System wide pathways including: - Social work consults - PT evaluations - Quality Assurance monitoring - Monitor and assess Geriatric Fracture Dashboard - Track quality metrics for all associated services (Ortho, MOTS, Social Work, PT, Anesthesia) - Monitor patient length of stay - Identify system issues - Consult with other hospital departments as appropriate to collaborate in patient care - Work with the Chief of Orthopaedic Trauma to implement and revise (as needed) clinical Requirements: Bachelor’s degree from an accredited PA program. Masters preferred. Current NYS Physician Assistant license and registration. NCCPA board certification   or   Master’s degree from an accredited school of nursing. Current NYS License as a Nurse Practitioner and RN. Board certification as a Nurse Practitioner. RNFA preferred. The Nurse Practitioner is required to enroll in RNFA program within 6 months of employment. The NP, without an RNFA license, must have 2 years of operating room experience at HSS. - DEA - BLS/ACLS certification - experience with an EMR (preferably Epic)

You will be responsible for ensuring research team is audit ready at all times by maintaining up-to-date regulatory documents and regulatory files.  You will also ensure the proper conduct of clinical research by developing, revising and maintaining accurate protocol tools at all times.   Responsibilities include, but are not limited to: - Manages all aspects of regulatory oversight for trials, from start-up through study closeout, and protocols in development. Works closely and collaborates with clinical research manager and PI to develop and complete necessary documentation to maintain studies - Supervises CRCs to ensure quality of data and regulatory aspects of studies. - Assists leadership in preparation of internal start-up documents, including budgets. - Assists in facilitation of CDA/CTA negotiations with PI, HSS legal team and sponsor. - Prepares Institutional Review Board (IRB) applications and submissions, including writing, reviewing and updating protocol logistics - Reports pertinent safety data and deviation notifications (Unanticipated Problem report) to the IRB - Prepares, reviews and submits – under directive of PI – DSMB, IND and NDA reports quarterly or annually. - Leads in development of protocols, consent forms, HIPAA authorization forms, other IRB-required forms and prospective databases specific to each protocol under the supervision of the PI. - Assures all site documents, including SOPs and administrative files, are maintained in audit-ready condition. - Performs pre-audit review of all regulatory documents for industry and investigator-initiated studies. - Reviews and adapts existing SOPs, templates, databases and organizational systems as developed for the management of regulatory and quality assurance activities. - Remains informed of current federal, state, and local regulations regarding clinical research and communicating any changes to study team. - House all CVs, medical licenses, certifications and ensure they are current. This includes notifying staff of impending expiration of various training or credentials. - Enters data into an electronic database. - Assists in the preparation of research presentations - Manuscript editing and preparation - Patient pre-screening and recruitment into clinical trials. Spearheads efforts to meet enrollment targets.  To that end, develops and implements various recruitment strategies. - Obtaining informed consent from patients who wish to participate in research studies - Manages scheduling and coordinating of QVs, SIVs, IMVs and COVs - Manages the scheduling and coordinating of study screenings, visits and follow-ups - Coordinates with research sponsors and other sites for multi-center studies - Participates in special projects as determined by management     We are seeking applicants who are effective multi-taskers, proven problem solvers and are detail-oriented. Applicants must have strong organizational skills, be good decision-makers, demonstrate flexibility in his/her approach, maintain a high level of professionalism and have excellent interpersonal and communication skills.   The qualified applicant will have a Bachelor's Degree in arts or science (BA or BS).  The ideal candidate will have 2- 5 years of experience in the field of regulatory documentation management or clinical research, experience submitting IRB applications (new and amendments) and extensive knowledge of Microsoft Office ((Word, Excel, and Adobe Acrobat, Tracked Changes functionality). Knowledge of clinical trial federal, state and local regulations preferred.   A two-year commitment is strongly prioritized.
