Complex Care Clinical Navigator

 

The Complex Care Clinical Navigator is a nurse with graduate level nursing education and training in the nursing model of care.  Autonomously and in collaboration with healthcare professionals and other individuals, the complex care clinical navigator provides a full range of primary, acute, and specialty healthcare services, including:

  • Fosters partnerships with all members of interdisciplinary team to facilitate a proactive approach to managing the care of complex patients to ensure optimal outcomes across the continuum of care
  • Working directly with patients and families and the interdisciplinary team to ensure a personalized plan of care based on identified needs and evidenced based assessments is implemented for patients with complex clinical needs
  • Serving as an advanced clinical resource and care coordinator for patients with multiple, complex medical conditions while offering education, support, and guidance to both patients, families, and staff

The Complex Care Clinical Navigator serves patients in an extensive range of practice settings throughout the hospital and its local and regional ambulatory sites, working collaboratively with all members of the interdisciplinary team to coordinate care for patients with complex care needs.

 

The Complex Care Clinical Navigator role:

  • Serves as a liaison between hospital departments and resources and coordinates the activities of members of the care team involved in the patients care (surgeon, hospitalist, physicians assistants, nurses, nurse practitioners, physical therapy, etc.)
  • Coordinates the interdisciplinary plan of care, as well as the timely scheduling of tests, treatments, and procedures to enhance the patient experience by helping to eliminate barriers to obtain definitive and timely access to needed care
  • Navigates and monitors complex care patients as identified across the continuum of care
  • Develops and facilitates goals and treatment plans in the pre-operative, inpatient, and post-operative settings

This Complex Care Clinical Navigator position is based at the main hospital in the Upper East Side of Manhattan (New York, NY). This is a full-time position, working ten (10) hour days.

 

This position requires a motivated individual who is able to function autonomously and in conjunction with all members of the interdisciplinary team.

 

Required:

  • Masters or doctoral degree from an accredited school of nursing
  • Licensure as an RN in the state of New York
  • CPR certification: BLS
  • Minimum of three (3) years of related experience within nursing in an acute care setting

Preferred:

  • Licensure as an advance practice registered nurse in the state of New York
  • Board certification as a Nurse Practitioner
  • Five (5) + years acute care nursing experience
  • Orthopedic experience
  • Experience with Epic
