Senior Director of Care Transitions (preadmission, admission, post-discharge)

POSITION SUMMARY

 

The Senior Director of Care Transitions is a Main Campus role, reporting to Inpatient Services.  The Senior Director is a nurse leader who will lead the Care Transitions including pre-procedure/treatment coordination, during the hospital/procedural encounter, and post procedure care coordination. The Senior Director of Care Transitions will have oversight of the Readmissions governance.  The Senior Director of Care Transitions is accountable for the overall direction, strategy, and operational execution of the plans for the delivery of high quality and safe care.  For this role, direct oversight includes pre/peri/post social work, multiple pre-procedure related programs, case management functions and all related programs, post discharge APPs and all related programs.  This role contributes to and executes the vision for promoting clinical excellence and program transformation in a highly competitive marketplace. The Senior Director of Care Transitions is responsible for oversight of relevant metrics including readmissions, insurance classification validations, same length of stay outcomes, overall care coordination, and is responsible for key partnerships.  The Senior Director ensures that high quality care and services are provided within regulatory and accrediting body standards, and that overall clinical outcomes and performance meet and exceed national best practice benchmarks. The Senior Director ensures clinical operations and all related programming functions at top levels of safety, efficiency, and with a patient centric focus on their procedural journey. The Senior Director is responsible for leading workplace engagement including integration of DE&I, team engagement and metrics around recognition, recruitment, and retention.  The Senior Director is accountable for viable financial performance of the areas for which the job has accountability.

The Senior Director of Care Transitions will lead a team of leaders who oversee social workers, RN case managers, discharge/transport coordinators, Post Discharge APP’s  and more, who are committed to serving patients with exceptional care and promote the mission, vision, and core values of Hospital for Special Surgery. These teams have touchpoints with patients before surgery, during the procedural encounter, and post discharge.  This is a leader of leader’s role, that requires experience guiding people leaders and teams to achieve key hospital metrics and institutional goals. The Senior Director of Care Transitions reports directly to the VP of Inpatient Services.

The Senior Director will partner and collaborate with nurse leaders, physicians, and several key stakeholders throughout Hospital for Special Surgery Enterprise to ensure the best patient care every patient, every time.

 

Qualifications

 

Educational Requirements

  • BSN from accredited school of nursing
  • Master’s in nursing or currently enrolled in a Master’s program

CERTIFICATION/LICENSURE - Required

  • Registered Nurse New York
  • Specialty Nursing Certification
  • AHA BLS certification required

 

EXPERIENCE - Required

Experience at the Senior Nurse leader, or Director-level, managing programming, clinical staffing, quality improvement programs, managing external partnerships; Experience with supervisory responsibly of a large headcount.

 

Years of Experience

  • 6 to 7 years and above of direct Supervisory/Management experience
  • Progressive nursing management experience coupled with relevant clinical expertise.
  • Experience managing stakeholders in a complex healthcare enterprise.
  • Project management experience.

SKILLS - Required

  • Excellent verbal and written communication skills.
  • Strong customer service orientation.
  • Knowledge of Magnet environment and regulatory standards; strong management ability and problem solving skills; ability to manage multiple projects and priorities simultaneously; Presentation and writing skills, ability to understand contemporary Performance Improvement processes, such as Lean methodologies and Performance Management; Must also be able to interact and negotiate with key stakeholders at the executive and physician level.
  • Knowledge of Lean methodologies i.e. Six Sigma, PDSA.
  • Knowledge of high reliability framework and tactics, continuous quality improvement.
  • Expertise in Office 365

Pay Range - Minimum

USD $180,000.00/Yr.

Pay Range - Maximum

USD $210,000.00/Yr.
